Diabetes in primary health care
The project intends to expand the evidence-based decision base for innovation in patient self-management competencies in diabetes and chronic conditions and to advocate the renewal of health care providers' professional delivery of primary care and home care services.
In line with the rapidly growing ageing population, as well as the increasing number of people with diabetes and other chronic conditions, maintaining high-quality health care services and ensuring proper workforce competencies are challenging.
The main objective of studies is to promote best practice research in diabetes and chronic conditions based on individual needs, and in implementing complex interventions and evidence-based guidelines.
